Achieving economic independence and social well-being in a supportive Minnesota neighborhood is our goal for the folks we serve.
Birch Homes LLC is a 245D licensed provider of respite, homemaker, Individual Home Supports (IHS) with training, and night supervision services. Clients choose a trusted, qualified worker for each 245D service, whereas for IHS with training, a qualified staff member is matched to the client to meet their needs. An assigned 245D coordinator will work with the client and the county case manager to set up and administer 245D waivered services.
Home and Community-Based Services (HCBS) standards are part of a larger Initiative to Improve people’s dignity, health, and independence. The guidelines for Housing Stabilization Services (HSS) are the same as those for HCBS. HSS services offered at Birch Homes are consulting, transitioning and sustaining.
