Birch Homes LLC is a 245D licensed provider of respite, homemaker, Individual Home Supports (IHS) with training, and night supervision services. Clients choose a trusted, qualified worker for each 245D service, whereas for IHS with training, a qualified staff member is matched to the client to meet their needs. An assigned 245D coordinator will work with the client and the county case manager to set up and administer 245D Waivered Services in all services.
The goals of the Home and Community-Based Services (HCBS) standards are to promote health, autonomy, and respect for clients. The HCBS standards that are outlined in Minnesota statutes chapter 245D are part of a broader effort to enhance client’s health, independence and dignity. The guidelines for Housing Stabilization Services (HSS) are the same as those for HCBS.
